A Clinical Study of Breast Cancer



Abstract
This clinical study was performed on 89 cases of the breast cancer that has been treated for 5 years from January, 1986 to December. 1990 at the Department of General surgery of maryknoll Hospital.
@ES The results were summarized as follow
@EN 1) All 89 cases were female. The follow-up study was 84 cases(94.4%) and the 5 year survival rate was 80.2%.
2) The most prevalent age group was 40 to 49 years(34 cases, 38.2%) and mean average age was 47.3 years old. In according to the age the 5 year survival rates were 89.5%, 75.3%, 89.5% at ages below 40, 40~59, and over 60 years, respectively.
3) The most common initial symptom (almost) was palpable mass and then axillary mass palpation(26.9%), pain(12.3%) and nipple discharge(11.2%).
4) The duration of symptoms of majority case(49.4%) was from 1 month to 6 months.
5) The most frequent location(60.7%) was the upper outer quadrant of the left breast(58.2%) was more affected than right breast(41.8%). In according to the location of breast cancer, the 5 year survival rates were 75.2%, 89.2%, 87.5% at
location,
outer
lesion, inner lesion & certral lesion respectively.
6) The most common size of tumor was a diameter of 2~5cm(37.4%). In according to the size of breast cancer, the 5 year survival rates were 100%, 81.0%, 63.5% at size, below 2cm, 2~5cm, over 5cm, respectively.
7) According to TNM system the most common stage of breast cancer as stage II(53 cases, 59.6%) and the least common stage was stage IV(1 cases, 1.1%). In according to the TNM stage, the 5 year survival rates were 100%, 92.4%, 51.9%, 0% at the
stage I,
II, III, and IV.
8) Axillary lymph node showed positive in 39 cases(45%). In according to the metastatic axillary node, the patients without metastatic axillary node had a better 5 year survival rates(88.6%) than the patients with metastatic axillary
node(68.9%).
9) The operative method was modified radical mastectomy in almost all cases.
10) Postoperative chemotherapy was performed in almost all cases, and postoperative radiotherapy were performed in 45 cases(50.6%). The 5 year survival rates were 88.2%, 71.4% at the chemotherapy only group and chemotherapy plus radiotherapy
plus
radiotherapy group, respectively.
11) The most common postoperative complication was fluid collection (10 cases, 11.2%).
12) The pathologic type according to the WHO classification was invasive ductal carcinoma (66 cases, 74.1%), medullary carcinoma(9 cases, 10.1%), invasive lobular carcinoma(8 cases, 8.9%).
13) Hormonal study was done in 46 cases. Premenopausal group was ER(+) in 10 cases(33.3%), PR(+) in 8 cases(31.3%). Postmenopausal group was ER(+) in 5 cases(31.3%), PR(+) in 4 cases (25%). In according to the hormjonal study 5 year survival
rates
were
70%, 87.5% at the ER(+), PR(+) in premenopausal group, respectively. And 100%, 100% at the ER(+) in postmenopausal group, Respectively.
14) The common distant metastasis was bone(10 cases, 11.2%), lung(7 cases, 7.9%), brain(3 cases, 3.3%), Liver (2 cases, 2.4%), contralateral breast(3 cases, 2.4%).
